I only accept the above health insurance. I can only accept a limited amount of insurance clients so please ask if I have openings for use of insurance benefits. 

Check with your insurance to see if you have massage benefits, how many, if you have met your deductible, & if you need a prescription from a physician or chiropractor.  Co-pays are required at the time of service.
